Spirit T-Shirt -- If you would like to pre-order a Spirit T-Shirt for your OCS student or yourself, please submit this form with payment to the PTA Mailbox.
Dollars for Dollars -- Buy your ShopRite, A&P, and/or Kohl's cards through the OCS PTA to earn us a 5% profit. Chair: Joanna Weldon
Check Request Form-- If you need a check for PTA purchases or to be reimbursed by the PTA, please submit this form (with applicable receipts) directly to the PTA Treasurer or send it to the PTA Mailbox in the Main Office.
Deposit Form -- If you have funds to deposit for the PTA, please submit this form with your deposit directly to the PTA Treasurer, Rich Michniewicz.
Mini-Grant Form -- Teachers may complete and submit this form first to Mr. Magnuson for approval, in order to apply for a mini-grant from the PTA.
